AR Student Reporting Services – Request Form

This form has been created to enable University staff and Students’ Societies to request reports concerning Tuition, Fees and Student Accounts records.

Depending on a request type, all fields of the request type section should be completed. Use the submit button at the bottom of the form to send your request to Student Accounts Reporting team. Once your form has been submitted, an e-mail will be generated with the contents of this request to you and Student Accounts Reporting team.

Note that all requests are subject to approval by the Student Accounts Office

Section 1: Requestor Information
Section 2: New or Ad-Hoc report needed
(Use this section to request a new regular to be repeated report or one-time ad-hoc report)
(should be completed only for a request of the new regular report)
Files must be less than 800 KB.
Allowed file types: gif jpg png pdf doc docx xls xlsx.
Section 3: Modification to existing report
(Use this section to request a modification to an existing report)
Files must be less than 800 KB.
Allowed file types: gif jpg png pdf doc docx xls xlsx.
